Enrollment Process

For individuals between ages 18-59, once a referral is made to Maximus, an appointment is set up with the individual within five working days from the date of referral to complete the initial assessment. In this visit, Maximus allows the individual to choose the top three service coordination agencies to coordinate support services. After the assessment is complete, within 30 days, Maximus coordinates with the physician’s office to obtain a Physician’s Certificate then makes referral to the local Area Agency on Aging (AAA) to complete a level of care assessment (LOCA), determining medical eligibility for waiver services within 30 business days from the date of obtaining the Physician’s Certificate. Once LOCA is completed, Maximus makes a request within 45 days to the local County Assistance Office (CAO) to determine financial eligibility.

In the case of individuals ages 60 and over, a referral from a participant needs to be made to the local AAA office in the individuals’ county. Within 5 days of the referral, AAA sets up an initial appointment with the individual. Within 30 business days from the date of initial assessment, AAA coordinates with the individual’s doctor’s office to obtain a Physician’s Certificate. Once the certificate is received, the AAA meets within 30 business days with the individual to complete the LOCA that determines medical eligibility for services. Once LOCA is completed, within 45 days, AAA makes a request to the local CAO to determine financial eligibility.

Once the waiver is approved by the Office of Long-Term Living (OLTL) or AAA and if Brighter Tomorrow Service Coordination is chosen as the individual’s Service Coordinating agency, one of our Service Coordinators meets with the individual to complete the paperwork on offering choices of direct care providers and coordinating additional services to meet the needs the individual.
